Christmas Week
This week is christmas week, and it's my on call week as well. On call? yea, when the Dr or staff nurse phoned you, he/she will tell you, r u tdm on call, janice kan? then she will say, there is a PCM poisoning case here in female medical ward 2. Can you come over to do the tdm? ok. So i will then ask her, how many pcm tabs did the patient ingested? then she said: 10... ok. so, i need to do then? she said: yes. ok. erm. 1st time being phoned up at 2am in the morning, in the super blur condition, i will definitely say ok, i will go. But 10 tablets isn't sufficient to kill the patient herself, and the tdm level tested will definitely will LOW. 2am drive to the hospital, and hospital isn't that near to my house... arrive there at 2.30am... do the level and finished quite fast though! wrote the on call time in the on call book for pharmacist. Then to claim for time off. A good experience to get up in the middle of the night and drive to hospital. Next day another on call @ 6.30pm. This is early. Patient ingested 10 tabs (again) and i needa test for the level again! Since it's still early, n my fellow tdm prp has promised me to go with me as beforehand we had agreed to be back in hospital for a urgent digoxin sample. Ok. but this call is a pcm call, hoping that the urgent digoxin blood sample can be sent to the hospital from hospital kota marudu in time so that i do not need to drive twice to the hospital. Pick up my fellow tdm colleague aka my good fren to go with me. And she brought her dinner with her so that she can have it in the tdm room. This time, level measured is still below toxic range. and luckily the digoxin sample arrived in hosp queen in time! so that i no need to go back hospital for another time. Spent like around 2 1/2 hours in tat room cause of some problems. 1st, that blood sample for pcm was taken @ 3 hrs 30mins post ingestion, which is of wrong sampling time. and i phoned to Female medical ward to ask the S/N to faster go n ask the on call doctor to take another blood sample. and thanks to the nice S/N that she managed to ask the Dr to take another blood sample in 1/2 an hour time. On call week is a stress week. They will call you at any time and u must be super alert!
Christmas eve, my brother organised a bbq and i ajak some of my colleagues to be here at my house! That day was my 1st day of on call and i am super scared that i will be asked to be there in hospital. But don care, after that bbq dinner, we went out for movie... and a really scary situation happened @ 1.15am where me and my friend were in the car to be back home. suddenly there is a little boy appear beside our car and he was asking for money for food. We r so scared ... lcukily my fren had locked the car door right after we pop up into the car. And luckily that boy din kick our car or so. Scary .. n luckily he went away when we don peduli him! huh... scary!!!
Ok. Week in tdm.. n next week is my last week to be in tdm... later will be going to tpn! oo..

Something to update!
Hello everyone. Today is friday n it's weekend again. Was in opd (outpatient department) during the last 4 weeks and is going into TDM (therapeutic drug monitoring) department next week for 1 month time. After my posting to TDM, i will need to start to have on-call for PCM poisoning!
Life in opd had been hectic but it's nice as i have a bunch of prps to talk to, to play with, and the time passes very fast! PA (pharmacy assistant) was nice too, and the bosses are good too! Some patients are just so nice and they love to talk to you! n some are so lame!
Last wed was my very unlucky day! 1st: early early morning, a patient approach to the counter when i pressed her number, and her 1st ques is: WHAT IS IFG? (stated in diagnosis of prescription) n then, i was like, huh, i dunno oh! Start asking my colleague beside me and asking all those experienced PA. No one knows though.
Then my answer to her is: I am not very sure.
Then she start bla back: What? you dunno??? you should know. You are the pharmacist and you should dispense the medications according to the disease!
Me: But i know your medications are indicated for hypertension as one of her diagnosis was hypertension as well.
Me again: Did u ask the doctor what's your disease?
She said: No oh. I just realized this "IFG" this morning. (as this is a repeat prescription).
Me: So do u wan me to check for you? so, just wait for a moment then.
So.. i go into the room, asking my bosses. No one knows oo. then, boss ask me to ask DIC ppl. n DIC ppl gave me an answer of : IFG: inferior frontal gyrus (a structure of the brain in between cerebrum). Ok. Cos there isn't any internet connection in opd, so i can't search it online! If search online, i just need to type IFG and i can find out the answer already! so, go back to the patient and tell her the same answer DIC ppl told me! (jus found the correct answer to that abbreviation, should be impaired fasting glucose)... haha
2nd unlucky thing of the day: Was being inspected by the audit ppl (what they called the procedure of ISO).. dispensed for 3 patients and auditor was just standing beside me.. so gang cheong... n did something memalukan oso.. lol..
3rd unlucky thing of the day: Patient prescription was not counter signed, n asked patient to get back to the doctor n counter signed. So... kena marah by the patient and said he wil phoned to the hotline and sue. saying why he needs to go to find specialist and get countersign while the prescription had been signed by a pegawai perubatan! So what. Pegawai perubatan can be just a HO, they don hv the power to prescribe you such expensive drug. and that's the procedure of getting countersigned when u want to have expensive medications. Who cares u ever took atorvastatin in hospital labuan, but currently u r in hosp QE, pls don mix things together! haiz! n he said he is going back to labuan at 4pm while that time was just 1.45pm. So, can't you jus move ur ass n go to get counter sign instead of waiting there n cari pasal dekat sana! His presence just make everything became mess! long queue behinds and he is jus buat bising there! u have so much money to take flight to labuan, don u jus buy those medications urself outside! n u came to QE just for the medications or u r seeking specialist advice? but the one u went for isn't specialist too! haiz... nothing to say!
that day as well, went to Sutera harbour Magellan for talk and had very heavy dinner! this sat got another drug talk n next fri got another talk again! .. .. . . . so nice oo..
